The version of FF4 Ultima I’ve designed is called “Ultima Iconic” because it offers many more icons throughout the game interface (as well as nicer graphics of characters and monsters).
The changes to items in the Ultima code show elements before the items that do specific elemental damage, and status icons before those which are associated with status removal or infliction.
These changes begin at 0786ED.
The spells are immediately after the items ( 078906 ), although some special magicks only found in FF4 Ultima are located elsewhere.
The above changes are current as of Ultima Iconic v2. “Time” in no longer offered, and that icon is instead the traditional grey orb from FF4, which is used for Summon spells without elemental affinity that don’t have healing effects.
